of the Hebrew Home of Greater Washington , our long-term care
community, located at 6121 Montrose Road, Rockville, MD OUR SOCIAL
WORKER POSITION The primary focus of the Social Worker role is to
coordinate resident's stay through the admissions, discharge and
MDS process. Provides crisis intervention and assists families in
understanding the implications and complexities of the life
situation and its impact on lifestyle. Provides professional social
work assistance to clients and families as related to illness,
disease, hospitalization, financial and educational wishes by
utilizing a variety of modalities in all areas of service. This is
a FULL TIME Salaried position with a M-F schedule. SOCIAL
WORKER-JOB REQUIREMENTS Assists the Resident in the exercise of
his/her rights and informs the Resident of any responsibility’s
incumbent upon him/her in the exercise of those rights.
Demonstrates the ability to obtain information and interpret
information in terms of the needs of the age of the Residents cared
for. Demonstrates an understanding of the range of care needed
based on the age of the Residents cared for. Demonstrates knowledge
of growth, development and/or adult health throughout the life span
as appropriate to the job performed. Excellent communication skills
both written and verbal. Ability to respect all members of the
healthcare team. Strong documentation skills. Solid work history
with excellent attendance and punctuality, patience, understanding
and compassion for those requiring our care. Provides considerate
respectful care focused upon the Residents' individual needs.
SOCIAL WORKER-EDUCATION & TRAINING Must have a License -Bachelors
in SW OR License Masters in SW- active license in MD. Completion of
a social work internship is required. Sufficient experience in
order to perform daily tasks with an understanding of the duties

is a Titer or Lab test that expresses proven immunity to the
following diseases. Upon employment our Talent Acquisition team
will provide additional details regarding CESLC vaccination policy.
