Together, we serve approximately 800,000 Kaiser Permanente members in Maryland, Virginia, and the District of Columbia at 34 area medical centers, plus several community hospitals and skilled nursing facilities.Job Summary:MAPMG is seeking a Cardiovascular Coordinator to provide support to the Cardiology department by scheduling patients admitted to the hospital for procedures, coordinating hospital transfers for complex patients, and then for their care after they are discharged with cardiac, vascular or dialysis teams.This position will be primarily remote with the ability to travel to the Kaiser Permanente Tysons Corner Medical Center on an as-needed basis.Responsibilities:Attends meetings, charts, and plans out care with the care teamDiscusses follow ups, cardiac, vascular, dialysis or other as neededNavigates scheduling procedures:Speaking with catherization lab to identify timesCoordinating anesthesia and verification of availabilityEnsuring device representation attendanceVerify cardiac interventionalist availabilityNavigates transfer of patient to core hospital:Coordination with transportation teamsCore hospital bed coordinationAll other elements ensuring a smooth transitionAll items needing coordination of the procedure listed aboveCoordinate hospital transfers for complex patients requiring advanced care for CHF, cardiac surgery, vascular surgery, or cardiac proceduresCoordinate transition in care for patients leaving the hospital to re-engage them with the KP cardiac or vascular surgery care team.Coordinate care delivery for patients awaiting dialysis accessCoordinates presurgical clearances with internal medicine, cardiology or by other specialists and PEECHelps with presenting outcomes on:Reduced time spent waiting for procedures when already hospitalizedReduction of physician time in coordination and navigation of careReduction of non-core hospitalization time and procedural volumeIncreased time to careCoordinates follow up with MAPMG cardiologist, vascular, dialysis and/or PCP after dischargeSets up ad hoc meetings for patients who are to be discharged or will need additional follow up with either MAPMG cardiology or other specialists and primary care physiciansUpdates records in KPHC, so team is aware of everything that is going on with the patient in real timeEngages in continuous communications with cardiology and vascular teams to make sure that they have all the records that are needed for a full evaluationEnsures pre-surgical evaluation by MAPMG specialists prior to surgery and after surgeryTravel between medical centers and hospital when necessaryOther vital tasks and responsibilities as needs ariseQualifications:Bachelor's degree or higher in Healthcare related field preferredMinimum of three years of relevant experienceProficient with word processing, PowerPoint, ExcelStrong project management and organizational skillsStrong written and oral communication skillsAbility to work with minimal supervision and in self-directed mannerAbility to maintain meticulous recordsAbility to interact on with medical staff, physicians, and patientsCompetitive Benefits:Competitive compensation packageComprehensive benefits including 100% employer-funded medical and dental insurance premiums for employees and familiesGreat work/life balanceGenerous paid time offMaternity and parental leavePension plan and 401(k) retirement planLife insurance, and short-term disability and long-term disability coverageEducation reimbursementEquity, Inclusion, and Diversity:MAPMG continuously works to identify and mitigate healthcare inequities, and that starts with providing an inclusive, supportive environment for our physicians.
