Job Description Job Description JOB SUMMARY This position is
responsible for delivery of appropriate occupational therapy
evaluation and treatment to PACE participants on acute, restorative
or maintenance levels as needed. Promotes professional working
relationships with both internal and external customers. Adheres to
and supports all organizational policies and procedures and
standards. Promotes teamwork! ESSENTIAL DUTIES AND RESPONSIBILITIES
Organizational (Core Rating Factors) Ensure discretion with
confidential information. Maintains courteous, helpful and
professional behavior on the job. Will support the success of the
entire team by promoting a collaborative work environment. Adheres
to all PACE of West Baltimore Policies and Procedures, PACE of West
Baltimore Safety Policies and Procedures and OSHA safety
guidelines. Consistently contributes to problem-solving and
cooperates with identified resolutions. Must demonstrate regular
attendance and punctuality. Brings issues and process improvement
ideas to the attention of the Supervisor. Maintains verbal and
written skills required for the position. Attends meetings and
participates in committees as required. Completes annual trainings
as required. Adheres to Company Dress Code Policy. Demonstrates
appropriate level of time management in support of co-workers and
the entire team. Departmental Essential Job Functions Serves as a
required member of the Interdisciplinary Team and attends IDT
meetings daily. Performs timely initial and annual assessments for
all participants and corresponding plans of care. Performs
semi-annual reassessments and unscheduled change of condition
assessments for all participants receiving occupational therapy
services as well as corresponding plans of care. Conducts home
safety evaluations as directed for potential enrollees of the
program Responsible for updating care plans and measuring the
outcomes for occupational therapy goals within care plan at least
every90 days. Assists in the delegation of therapy plans for
specific participants to Occupational Therapy Assistants according
to complexity of participant’s needs and nature of required
occupational therapy procedures. Trains family members, caregivers
and other staff members in methods to enhance mobility, safety and
functional independence of participants. Assists in the ordering of
durable medical equipment for participants. Trains family members,
caregivers and other staff members in methods to enhance mobility,
safety and functional independence of participants. Fulfills
company documentation requirements. Additional responsibilities
will be added at managers discretion, as career growth
opportunities and responsibilities are constantly arising in our
flexible and start-up environment. QUALIFICATIONS Graduate of an
occupational therapy program accredited by the American
Occupational Therapy Association. Must have excellent written and
verbal communication skills and demonstrate the ability to interact
clearly and effectively with both internal and external customers.
Must submit to and pass a pre-employment drug/alcohol screening and
criminal background check. Must have a current Driver’s License and
driving record must be in compliance with Company policy. Must
maintain current automobile insurance. Must have working knowledge
of computers and Microsoft Office applications Skilled in
establishing and maintaining effective working relationships with
participants, co-workers, vendors, medical staff, and the public.
Must be free of communicable diseases that can be readily
transmitted in the workplace. TB test is required within the first
7 days of employment. HepB vaccinations are offered but not
required. Operates in full compliance with all applicable federal,
state and local fire, health, safety, sanitation, Emergency
procedures and other Standards prescribed in law or regulations.

working under stressful conditions. Moderate pressure to meet
