Documents in the medical record using the Academy of Nutrition and Dietetics’ standardized language as part of the Nutrition Care Process. Performing nutrition assessments and identifying nutrition diagnoses; recommending and implementing nutrition care plans based on evidence-based practice. Identifies appropriate nutrition diagnosis and malnutrition criteria. Identifies age-specific physical, psychosocial and cultural needs when carrying out patient activities. Working collaboratively with a multi-disciplinary healthcare team to promote wellness, maintenance of current health, and/or intervene in acute or chronic illness. Communicates the nutrition plan with the interdisciplinary team. Providing education at the appropriate level/learning mode for the patient/family and documenting a learning assessment and an assessment of patient’s readiness to learn, teaching methods, and response to the information provided. Knowledge of food science, biochemistry, physiology, management science, behavioral science and social science as applied to human nutrition is required. Must be credentialed as a Registered Dietitian (RD) by the Commission on Dietetic Registration and licensed as a Dietitian-Nutritionist (LDN) in Maryland. Entry-level will be considered.
