Job Description:

A company is looking for a Clinical Care Specialist.
Key Responsibilities

Manage and monitor a patient census, implementing Chronic Care Management services
Create individualized Care Plans and track patient progress, including labs and medications
Coordinate care with patients, families, and healthcare teams through various communication methods

Required Qualifications

Registered Nurse (RN) license in applicable state
Five years of nursing experience required
Experience with chronically ill patients preferred
Telephonic case management or care coordination experience preferred
Strong computer skills and reliable internet connection required
