Job Description:
A company is looking for an LPN/LVN Health Navigator.
Key Responsibilities
Monitor and screen incoming alerts, calls, and chat
communications
Conduct patient onboarding and assist in reviewing patient
records
Communicate patient concerns to the Registered Nurse and
troubleshoot technology issues
Required Qualifications
Current, active LPN/LVN license in California in good standing
Minimum of 2 years hands-on nursing experience; telehealth
experience preferred
EHR experience, specifically with AthenaHealth, is a plus
Ability to work collaboratively in a remote healthcare
environment
Strong computer skills and ability to learn new technology
